  • Information regarding professional activities within work – e.g. positions of responsibility, involvement with professional working groups – C.E. / special interests ....
  • List of rotations/dates/line managers/responsibilities
  • Record of courses attended/certificates/course timetables, alongside evaluation sheets
  • Reflective practices
  • Record of in-service training: could include who led training/your involvement/summary of topics covered/application of learning into practice.
  • Evaluation of literature – critical appraisal documentation e.g. journal club activity
  • Audits/case studies

Remember – this list is not exhaustive and is only for guidance – it is up to each individual what they do/do not wish to include in their C.P.D. record
